1	All the fields with * are mandatory.
2	Please upload a photograph and signature not exceeding 100 kb in size and Date of Birth, Category Certificate (if applicable), Qualification, MCI Registration MBBS, MCI Registration PG, MCI Registration Post Doctorate and Experience Certificate in the form of PDF/Image. The size of each scanned pdf should not exceed 1 MB each. The candidate must ensure that their photo and signature should be clearly visible in preview at the time of filling of application in online mode.
3	After primary registration, you will receive a confirmation E-mail. This confirmation e-mail will also have applicant ID and a password to login into the system and fill in the application form online.
4	All the further communication will be made through your registered email address so please enter a valid email address.
5	You have to login into the system to complete your application. Your application will be accepted only after receiving the payment. Application having pending payment deemed to be rejected.
6	All Payments should be done though online mode only.
7	You can add personal information only once. Once you have added your personal information, add personal information button will get disabled.
8	The applicants applying in response to this advertisement should satisfy themselves regarding their eligibility for the post applied for. They must be fulfil the eligibility criteria as well as experience as on the closing date of application failing which their application will be rejected.
9	Please do not check the declaration unless until you have completed your entire form once you submit the declaration your form will be finally submitted and you wont be able make any amendments.
10	Application fee once paid shall not be refunded under any circumstances.
